Evaluate a Coach

Dear Parents and Players,

To help us evaluate the effectiveness of our coaches, we would appreciate if you could take a few minutes to complete a questionnaire about your soccer season.

We ask that parents and players complete separate questionnaires, and that you submit a separate questionnaire for each coach and assistant coach.

Unless otherwise chosen all surveys will be submitted anonymously.

Option Number 1
Submit Online -  click here to log into our system. 
(Each family who has provided an email address to the club, can access the sytem using the email address of their 'Designated Contact'.  If you have not previously provided the club with an address and wish to do so now please email your name, phone number and email address to registrar@cswusoccer.com  .

Option Number 2
Download the evaluation form, fill it out and mail it to the address which is on the form.  Click HERE to download the evaluation form.
